1. How can I make an appointment as a new patient?

2. What do I need to bring for my visit?
Your insurance card, copayment, your current medications list and a photo ID.
3. How can I get my prescription refilled?
You can call our office and leave a message on our prescription refill line. Please allow two business days to complete a prescription refill request to be called into your pharmacy.
4. I am sick, how soon can I get an appointment?
We do save some open slots on our schedule for sick visits. The appointments do go quickly therefore please call as early as possible so we can fit you in.
5. With which hospital is Riverside Medical Group affiliated?
Lowell General Hospital, 295 Varnum Ave, Lowell, MA.
6. Where can I go for blood work?
Lowell General Hospital or any laboratory or hospital that your insurance will cover.
7. Preparation for blood work
Depending on what kind of blood work you are having, you might need to fast for 8-12 hrs. Please ask your healthcare provider when they give you the order form, or call the office if you are unsure.
8. How do I get a Referral?
You can submit a referral request on our referral request line. Please check with your PCP before you request a referral for a new specialist.
9. How do I obtain my medical records if choose to move to another PCP?
You must fill out a Medical Records Release Form or you can get one at our office. There may be a fee for processing your records. A copy of your records will be available for you to pick up at our office.
